Your union has entered into an agreement with PSS to provide reduced rates for custom orthotic appliances.
The Union currently provides health insurance, HSA, or other third-party benefits that may cover some if not all of your appliances under your Union Benefit Plan. As a result of the preferred provider agreement with PSS, union members and in some benefit plans, their families, will now have the opportunity to receive PSS orthotics from a PSS preferred provider, annually or bi-annually depending on your plan.
The Provider will perform an examination, take a cast impression, and provide a prescription for the orthotics. Depending on your Union agreement, The Provider will charge all applicable office visits, x-rays, and surgical fees directly to the insurance. The Insured will be responsible for the co-pay
Orthotic orders for Union members will be sent exclusively to the PSS lab.

To get fitted for your custom-molded orthotic
The Union agreement requires a voucher. The voucher entitles the patient to be cast for custom molded orthotics and receive one pair of these devices from a conveniently located PSS Preferred Provider. The cost of the voucher is determined by your union or affiliate. After payment is received, a voucher will be sent to the patient. Vouchers may be redeemed in the Preferred Provider office at the time of casting for the orthotics. The patient is responsible for office visit charges and all reasonable and necessary other charges including x-rays and surgery. If the patient has Blue Cross/Blue Shield, Personal Choice, or Medicare, the insurance will be billed by the doctor and accepted by the Insurance company. The patient is responsible for co-pays and deductibles. If the patient has HMO coverage, he can either go to his doctor for an examination and a prescription for orthotics and then visit the PSS Preferred Provider for casting & dispensing of orthotics or, he can pay the PSS Preferred Provider for all services that are not covered by the prepaid voucher. When the orthotics are ready, the patient will go back to Preferred Provider and will be fitted for the orthotics.

A prescription pair of custom molded orthotic appliances, every 24 months (2yrs), will be paid directly to Pro Support Systems (PSS) by the Union benefit plan.
As a result of the preferred provider agreement with PSS, the Union members will now have the opportunity to receive PSS orthotics from a PSS preferred provider and are not responsible for any balance of the cost of the device which includes the impression casting and office visit, except for a $20 co-payment.
Many of our finest Doctors nationwide do not subscribe to HMOs. If the member wishes to care for non-orthotic services, he/she may request this and be responsible for the fee. He/she will be advised that the provider is not enrolled in Keystone although most are in Blue Shield if not all. They can bill for services to Blue Shield that are covered and they must accept assignments as Blue Shield Doctors. It will be the informed choice of the patient. The fund is not responsible or liable for payment or coverage.

Orthotics are paid through the coverage of the HSA Union-issued credit card. Additional podiatry services and examinations, and office visit charges are billable to Independent Adminstators Coverage also provided by the Union.

As a result of the preferred provider agreement with PSS, the Union members and their families will now have the opportunity to receive PSS orthotics from a PSS preferred provider and are not responsible for any balance for the cost of either the device or the casting.
